In vivo, the body converts dietary folate and food folate into 5-MTHF (also known as l-methylfolate or 5-methyltetrahydrofolate) through a multi-step process, in which methylenetetrahydrofolate reductase (MTHFR) plays a key role. However, unlike food folate, folate must first be reduced to tetrahydrofolate by a two-step reaction of dihydrofolate reductase (DHFR), and then further metabolized.

Due to the unique genetic pattern and expression, MTHFR enzyme activity decreased in some individuals. They have the polymorphism of this enzyme, which is related to the decrease of plasma level of bioactive 5-MTHF folate.
